1- Brazil Visa Application form completed and signed.
2- recent passport type photograph.
3- Copy of round trip airline tickets on travel agent letterhead or letter from travel agent confirming round trip tickets will be issued.

For Non  US Citizens
Bank statement needed from bank for sufficient funds For Non  US Citizens A copy of Green Card or Legal Status in United States Should also be Submitted
