make sure That all of the entered information is complete and correct before printing. Print the completed form using black ink on one-sided pages.
3- 2 recent passport size (2x2) photographs.
4- Letter of Authorization Print out sign and date this letter of authorization
5-Copy of flight itinerary or ticket
6- Department of State Processing Fee
7- A Washington Travel & Passport Visa Services fee
A-11-14 business days $55.00 plus + government fees.$135.00
B-5 business days $95.00 plus + government fees.$135.00
C-1-2 business days $125.00 plus + government fees.$135.00
8- Return Postage - $24.50 priority overnight (up to 3 passports) $37.50 (Saturday delivery)
9- Completed A Washington Travel & Passport Visa Services form.Passport Transmittal
10- All the above requirements should be taken to a main US Post office or a Circuit court where your application will be executed in front of the clerk. The clerk will enclose the information in a sealed envelope. Once it has been sealed it cannot be reopened.
