A valid passport with at least 6 months validity remaining.
One application form, fully completed and signed.
2 passport type photographs.Vaccinations - International Certificate of Vaccination for Yellow Fever required if arriving from infected area within 5 days
A notarized copy of the Green Card or a valid visa back to the U.S. if the applicant is not a U.S. citizen.
Proof of sufficent funds during the intended stay in Pakistan.
Onward/return ticket or confirmed itinerary.
Pakistan Business Visa.
A valid passport with at least 6 months validity remaining.

A letter of invitation from the host company in Pakistan.
A letter from the company in the U.S. stating the purpose and Duration of the trip Proof of sufficent funds During the intended stay in Pakistan.
Onward return ticket or confirmed itinerary.
Other Information.
Passport holders from India are required to submit their visa request on this application form.
International Certificate of Vaccination for Yellow Fever required, if arriving from infected area within 5 days.
