Each application must have a 2 color passport size photo attached to the application

2-2 A copy of the Green Card or a valid visa back to the U.S. if the applicant is not a U.S. citizen
Proof of hotel reservation or stay with relatives/ friends
Onward/return ticket or confirmed itinerary
