General Requirements for Replacing a Lost or Stolen Passport
Please prepare and submit the following documents:
🔷 Passport Application Form DS-11
- Complete the DS-11 application online
- Print the completed application with barcode
- Do NOT sign the application until instructed by a Passport Acceptance Agent
🔷 Proof of U.S. Citizenship
Submit one of the following original documents:
- Certified U.S. Birth Certificate
- Certificate of Naturalization
- Previous U.S. Passport issued more than 15 years ago
🔷 Passport Photo
Provide:
- One recent 2” x 2” color passport photo
- Plain white background
- No glasses or uniforms
🔷 Travel Itinerary
- Copy of your round-trip travel itinerary
- Flight confirmation or travel reservation
🔷 Government Passport Fees
Applicants Under Age 16
- $160 Passport Fee
- $35 Execution Fee (paid separately to acceptance facility)
Applicants Age 16 and Older
- $190 Passport Fee
- $35 Execution Fee (paid separately to acceptance facility)

Important Application Instructions
All required documents must be presented at an authorized Passport Acceptance Facility such as:
- U.S. Post Office
- Clerk of Court
- Public Library
- County Government Office
The Acceptance Agent will:
- Verify your identity
- Witness your signature
- Administer the oath
- Seal your application package in an official envelope
