Requirements for Replacing a Lost or Stolen Passport
Prepare and submit the following documents:
🔷 Online Passport Application (Form DS-11)
(Complete online and print with barcode. Do not sign until at the acceptance facility.)
🔷 Original Proof of U.S. Citizenship

Certificate of naturalization

OR a previous U.S. passport issued over 15 years ago
🔷 Passport Photo Guidelines
(One recent 2×2” color photo with a white background)
🔷 Copy of your Round-Trip Itinerary
(Flight tickets or a confirmed travel reservation)
🔷 Passport Fees for Minors (under 16):

$160.00 + $35.00 execution fee (paid to acceptance agent)
🔷 Passport Fees for Adults (16 and older):

Important Instructions
All the above documents must be taken to a Passport Acceptance Facility (post office, courthouse, or clerk’s office).
The clerk will administer an oath and seal your documents into an official envelope.
Once sealed, the envelope must NOT be opened.
