First-Time Passport
- Online Passport Application. The secure online application is fully completed and printed. (This form must be completed online at the Department of State website & will print with a unique bar code.) Only sign the application once you take the oath administered only by & an authorized passport acceptance agent. To find the nearest authorized passport acceptance facility
- Original proof of US citizenship. Certified birth certificate naturalization certificate, or passport issued over 15 years ago.
- Passport Photograph. 1 Recent Passport-type photograph (2″ x 2″) in color, front view with white background. For more information click HERE.
- Itinerary. You’ll need a copy of round-trip tickets or a confirmed itinerary.
- Passport for Minors. Under the age of 16: $160.00 + $35.00 Acceptance Fee to Post Office
- Passport for Adults. Age 16 and over: $190.00 + $35.00 Acceptance Fee to Post Office.
- Visa DC Submission Form.
- Visa DC Authorization Form Third-Party Release Letter. This is an authorization letter for release to Visa DC Services.
- All first-time or new applicants need to have their applications authorized by local government officials called acceptance agents us, post office, a circuit court,
