Second US Passport
- Online Passport Application. Could you complete the secure online application and print it? (This form must be completed online at the Department of State website and will be printed with a unique bar code.)
- 10-year Passport. A valid and signed US passport.
- Letter to the US Department of State, signed by the applicant, substantiating the need for a second passport. Sample Letter Template.
- Passport Photograph. 1 Recent Passport-type photograph (2″ x 2″) in color, front view with white background. For more information click HERE.
- Itinerary. A copy of round-trip tickets or a confirmed itinerary is needed.
- Passport for Minors. For those under 16: $160.00 + $35.00 Acceptance Fee to Post Office.
- Passport for Adults. Age 16 and over: $190.00.
- Visa DC Submission Form. Visa DC Service Submission Sheet.
- Visa DC Authorization Form. Third-Party Release Letter. Authorization letter for release to Visa DC Service.
