Prepare the following documents:

Important Instructions
All the documents must be taken to a Passport Acceptance Facility such as a post office, courthouse, or clerk’s office.
The clerk will administer the oath, verify your documents, and seal them in an official envelope. Once sealed, the envelope must NOT be opened.
