Requirements for a First-Time Passport Application
To apply, you must prepare and submit the following documents:
🔷 Online Passport Application (Form DS-11)
(Complete online and print with barcode. Do not sign until you appear before an authorized acceptance agent.)
🔷 Original Proof of U.S. Citizenship

Important Instructions
All first-time passport applicants must appear in person at a Passport Acceptance Facility (post office, clerk of court, or other approved government office).
The acceptance agent will administer an oath and seal your application materials in an envelope.
Once sealed, do not open the envelope.
