General Eligibility Guidelines for Passport Name Changes
You may qualify for a passport name change application if:
- Your current passport was issued less than one year ago using Form DS-5504 (government fees may not apply)
- Your passport was issued more than one year ago and you can provide legal proof of your name change using Form DS-82
- Your name changed due to marriage, divorce, or court order
Eligibility and approval are determined solely by the U.S. Department of State.
